Renfrewshire Telehealth Project

This report provides an evaluation of the pilot undertaken by Renfrewshire Council Social Work Services department to determine the benefits of using Telehealth in the health management of patients with Chronic Obstructive Pulmonary Disease (COPD) and of patients with non-specific conditions attending their GP surgery.

Argyll and Bute Telehealth Project

This report provides an evaluation of the pilot undertaken in Argyll and Bute.  Three types of telehealth initiative were trialled: home telehealth pods for patients with COPD on the Isle of Bute and Isle of Luing; surgery telehealth pods for more general patient monitoring in Oban, and Rothesay on the Isle of Bute; and community telehealth pods in Oban and the Isle of Luing.

Home Based Pulmonary Rehabilitation Project

The project is using a novel communication system to deliver group-based pulmonary rehabilitation in the home via the internet.  The project is a collaboration between Distance Lab and the Centre for Rural Health and has been funded by a £63,000 grant from Chest, Heart & Stroke Scotland (CHSS).  Click here to visit their website.

Inverclyde CHP COPD Work

In order to treat individuals with long term conditions more proactively, nearer home, and earlier in the course of their disease the Inverclyde CHP in partnership with Inverclyde Council, patients, carers and other relevant agencies has piloted a model for long term conditions that delivers care to match the level of need. A fundamental aspect of improving care delivery is the facilitation of multidisciplinary collaboration and effective information sharing. The telehealth model (the remote provision of healthcare support) described in this report is supported by Doc@Home technology which allows the sharing of patient details between multidisciplinary teams.
